Submitting your forms in advance allows our team to verify your benefits and prepare an estimate of any fees, co-payments, or coverage details prior to your appointment. Please note that patients are ultimately responsible for all fees and co-payments associated with their care. While we do our best to provide accurate treatment estimates for patients with out-of-network insurance plans, reimbursement rates can vary by policy and are not always available to our office. Because of this, estimates may differ depending on your individual plan.
We are always happy to submit a preauthorization to your insurance company to confirm benefit amounts and provide a more exact out-of-pocket estimate. Please note that this process typically takes 2–3 weeks to complete. During your consultation, our staff will also be happy to review our financial policy and answer any questions you may have.
